fakers. there is an astounding # of "fakers" on our unit. kids who PRETEND to have an illness. the scary thing is, many of them are extremely good at it. my first response whenever i hear of a faker is: maybe they ARE ill. let's give them the benefit of the doubt. my tendancy is just to take the safe side. if we get suckered, we might feel stupid, but if we dismiss too lightly, some kid is going to suffer b/c of our doubt. that's the way i try to approach things of this nature. but: case 1: kid complains of a shooting pain in both legs. says he can't walk, it hurts so much. however, on a neuro exam, he is able to press down on his feet, and lift his legs against resistance with quite a lot of strength, with no apparent pain. later, i observe him walking to and from the bathroom, again with no problems. case 2: kid complains of intense pain all over her body. on physical exam, she allows herself to be examined, with no discomfort or manifestations of pain anywhere. completely befuddled residents, until one smart resident came up with this: resident: "X, it looks like you have a SPECIAL condition." kid X: "really?" resident. "yep. you have all the symptoms. in fact, usually people with your condition see everything in orange. do you see things in orange?" kid X: "uh huh. you're orange right now!" resident: "and they also get a tickly sensation on the inside of their nose." kid X: "yeah! i've got that!" resident: "and they are also incredible liars!!" HAHHAHAHAHAHAHa. ok, the resident didn't say that. but she easily could have. because she was totally making up these symptoms, and the kid was agreeing to whatever the resident was suggesting. there are various reasons why the fakers do what they do. for attention, mostly. because of a new younger sibling. or a family member who was sick and got so much love and attn. or some just want to get out of school. amazing. they are so clever about it, too. esp when they know how to look things up on the internet. they look up what they are supposed to have, and are able to mimic very well. good thing that even internet info sites have limited information, and so we are able to catch their lies. ******* yakob at 10:32 PM |
